Apple saw a blank screen, spinner, or 'could not connect' error because your server was asleep, unreachable, or blocking the reviewer's region when they tested.
Apple said something like
We were unable to review your app as it exhibited one or more bugs. Specifically, the app displayed an error message on launch.
The app was unable to load content. When we tapped [feature], nothing happened / the screen remained blank.
We were unable to load your app's content. Please ensure your servers are available and try again.
The app displayed 'No internet connection' / 'Something went wrong' on a stable network.
We entered a valid code but the app returned 'invalid code' / could not find the item. The feature did not work as described.
